COVID-19 Daily Self-Survey

Parents and Staff: Please monitor for the following COVID-19 symptoms daily, prior to attending school. Please notify the school and contact your physician and/or Caldwell County Health Department for further direction, if you are experiencing symptoms. When symptoms seem to be escalating, or serious symptoms are present like difficulty breathing or chest pain/pressure, seek immediate attention; then have someone call to notify your physician, or the health facility assisting you, of your symptoms before you arrive. If symptoms are emergent, call 911.


1) Have you been in close contact with someone with confirmed or suspected COVID-19?

(Per CDC (Centers of Disease Control)—the definition for Close Contact is someone who was within 3 feet of an infected person for at least 15 minutes starting from 2 days before illness onset, or had direct exposure (e.g., direct coughing into face...)

If you answered Yes, to being exposed, even if you are not currently having symptoms, Stay at Home; please self-quarantine for 10 days following contact, let your school know you will not be there, and contact your personal physician for recommended care.

2) If you answer No to question 1 above (has No known exposure to COVID-19), continue to daily symptom checker below.

*If you are experiencing a high-risk symptom, or vomiting, or diarrhea, please stay home until 24 hours after your symptoms have resolved. Call your physician for further care recommendations.

*If you are not experiencing any high-risk symptoms, but have 2 or more other symptoms listed, that are not ‘normal for you’, please stay home until 24 hours after symptoms have resolved. (e.g., ‘Normal for you’ such as seasonal allergies…)

High Risk COVID-19 Symptoms With no Known Exposure to COVID-19 or other Contagious Illness-- Any symptom is Mandatory 24 hour exclusion.

* Fever of 100.4 or higher

* New Cough

* Shortness of Breath or Difficulty Breathing

* New Loss of Taste or Smell


Other Contagious Illness Symptoms-With no Known Exposure to COVID-19 or other Contagious Illness-- Any symptom is Mandatory 24 hour exclusion.

* Nausea/Vomiting

* Diarrhea

* Unexplained Rash

Other Possible COVID-19 Symptoms-If has 2 or more symptoms with no known exposure to COVID-19 or other contagious illness-stay home and may return when symptoms have resolved for at least 24 hours.

* Fatigue

* Muscle or Body Aches

* Headache

* Sore Throat

* Congestion or Runny Nose

*If your student is having symptoms, and you are unsure of what to do, please error on the side of caution by keeping them home, and contact the school for further direction.
